Did something happen to you at school?
Did you witness something happen to another student?
Do you have something important to report to adults about the safety of students and staff in our school?
Please use this form to report any incident that you want the school to be aware of. Examples: Bullying, Harassment, Racism, theft, weapons, drugs, fighting, peer conflict, or any other safety concerns. This form may be completed by a student, staff member, parent, volunteer, or community member.
Important Note: We do not disclose the name(s) of our sources. We are sensitive to the confidentiality concerns of students and parents/guardians. If you or your student experiences retaliation of any kind, it is important that you contact a school administrator so that they may appropriately address the concern.
